FENTANYL CITRATE (SUBLIMAZE)

CLASS:

  • Narcotic (Opioid) Analgesic (Schedule II)

ACTION:

  • CNS Depressant, Binds to opiate receptors producing analgesia and euphoria

  • Reduces the brainstem respiratory centers responsiveness to CO2, also depresses pontine and medullary centers involved in regulating respiratory rhythmicity thus produces respiratory depression.

INDICATION:

  • pain control

  • maintenance of analgesia and adjunct to RSI

CONTRAINDICATION:

  • Hypersensitivity

  • Respiratory Depression

  • Myasthenia gravis

STANDARD PREP:

  • 50 mcg/ml

    (80-100 times as potent as Morphine Sulfate)

DOSAGE/ROUTE:

  • 50 – 100 mcg/dose (0.05 – 0.1 mg) IM or IV/IO

    (slow 1-2 min) max. dose 150 mcg (0.15 mg)

  • Onset: Immediate — Peak: 3-5 min (IV) — Duration: 30-60 min

PEDS DOSE:

  • 1 – 2 mcg/kg IM or IV/IO (slow 1-2 min) [Need to contact BSP]

SIDE EFFECTS:

  • Respiratory depression

  • Hypotension

  • Dysrhythmias

  • Palpitations

  • CNS depression or euphoria

  • Dizziness

  • N/V

PRECAUTION:

  • Use with caution in patients with traumatic brain injury

  • Serious respiratory depression can be reversed with naloxone (Narcan) if necessary.
